a. I believe that the advocacy approach dominated the decision making process in the Challenger case. The Morton Thiokol engineers failed to communicate with the NASA engineers about the dangers of liftoff when the temperatures were below fifty-eight degrees Fahrenheit. There was an inordinate amount of pressure on the engineers to go ahead with the liftoff despite the apparent risks. I believe that Edward DeBono’s philosophy on parallel thinking could have averted the Challenger disaster. If the engineers had taken the time to view the O-ring problem from the various perspectives that DeBono emphasizes in his six hat model, I believe that disaster could have been averted. The Challenger disaster points out the various hidden traps in decision making. The engineers were content to go along with the status quo. They viewed liftoff as the only alternative. There were other alternatives to the status quo available, such as delaying the launch until the O-rings could be inspected, but the engineers chose not to examine the alternatives.

b. The advocate and inquiry approaches is a critical part of the decision making with an agreement made on the readiness of the Challenger to be launched in space. The Challenger disaster points out the various hidden traps in decision making. The engineers were content to go along with the status quo. They viewed liftoff as the only alternative. There were other alternatives to the status quo available, such as delaying the launch until the O-rings could be inspected, but the engineers chose not to examine the alternatives. The discovery and collaboration orientation is an important part of the decision- making process and it will decrease mistakes and traps. It allows individuals to stay focus on the issues that need to be addressed and approach resolution.
